Dupe bug:
Draconic infused obsidian and wither proof glass from runic dungeon will dupe when hit by the wither. I've gone ahead and trashed the duped blocks, but be ware that it happens. -
Known bug, no current solution.
As long as there isn't abuse it shouldn't be a problem, and since it's ultimately just obsidian and glass it shouldn't be too big a deal. -
Very true, it's just a pain to have you inventory getting filled with it, it has no other use than to shield from the wither

Dupe bug:
Black hole talisman with a block inside in an ender IO crafter will dupe the block one stack at a time. -
report it here, but since 2.3.5 is the final pack version it will never be fixed
